• It happened before and was resolved after we upgraded to the latest version, last Thursday it happened to us again, do you know what could cause it?

Unfortunately, I did not meet with such situation myself.

If you will find how to repeat the issue and can provide access to the development copy, where the issue is repeatable, I’m ready to do my the best to investigate it and provide the fix ASAP.

I need a constructive help. Can you provide any details, scenario?
    How to reproduce the bug?
The problem is that it’s the definitely rare bug, if compare the general quantity of active installations (>700 000) with 2-3 similar reports made recently.

Recover – restore the values for this record from the latest backup copy of the database.

Any clue/thoughts, what user exactly did before got this result?

We did not create a new user and did not update an existing user.
We have not changed capabilities for any user or role.

It this case, why are you sure that URE erases user roles data, if you even did not make any changes using it recently?

I recommend to check the WordPress core, plugins, themes files for versions installed/modified without your permission. Sometime site may be hacked by intruder. It would be good to have the older backup for which you are sure that files are clear – to make the automatic files comparison.

Because it happened to me on some sites that all have your plugin installed, it did not happen on sites where the plugin is not installed, and in the past it was resolved after we upgraded to version 4.59.1
