the plugin does not work properly

Hello,

I have a problem on my website with your plugin. This error occurs when the administrator receives the emails with the information that a customer has put in a form. This information is not able to display well because you do not get to see the user’s name, subject, or message. This happens with the two forms that are published on my website, one located on the contact page and another one on each of the pages of the courses we offer. I attach a screenshot of what we get when a customer submits a form.
