text/plain being sent for W3TC requests through admin.php

A content-type of text/plain is being sent with the page in the popup from clicking any of the links in “Make sure to export the media library and upload the wp-inludes”. This of course prevents the page from working. An example URL:
Requests through admin.php from other plugins work as expected and have a content-type of text/html.
This happens on Apache in my local development environment (Vagrant/Ubuntu) and in the production environment (Amazon Linux).
I’ve disabled all other plugins and disabled every W3TC option other than CDN and I’m still getting the behavior.
What could be causing this behavior?
