Text Input Not Being Recorded On Submission

    I have a radio button field with ‘No’ and ‘Yes’ options. Using conditional logic when ‘Yes’ is selected a checkbox field with several options will show. When the radio button is ‘Yes’ and the checkbox field has ‘Other’ selected a text input field will show. When I type in the input field and successfully submit the form, the text input field is blank in the submission data.

    Apologies for the inconvenience here!

    We’ve tested the form and found the issue which was causing it, it’s still not fully clear why this is happening, but we’ve found a temporary solution.

    Please try this:
    – edit the Allergies checkbox
    – switch to “bulk edit”
    – change all uppercase letters to lowercase like this:

    cantaloupe;cantaloupe; 0
honeydew;honeydew; 0
pineapple;pineapple; 0
strawberry;strawberry; 0
mango;mango; 0
fish;fish; 0
cheese;cheese; 0
tree nuts;tree-nuts; 0
peanuts;peanuts; 0
other;other; 0

    – test and see if that worked

    There may be a fix coming for this or a similar issue in an upcoming update (it’s already in tests), but I will report this as well to the team so they can check this further.
