Text box not included on page

Hi there,

I’m trying to create a pretty average comment box on my website. I created a custom contact form using the plugin and successfully put it on one of my pages by including [customcontact form=1].

My problem is that on the actual page the empty text box is not appearing, only the title script and the submit button.

I filled out a new field with the “text” type under the “field” tab. What do I do to connect this to my custom contact form?
