Text and checkbox when submitting

  • Hi there,

    Consider me a newborn at this!
    I’m trying to add a text, sort of Terms and Conditions under a link and a checkbox next to it, under my submit form.
    HOW?!

    Any kind of help/reference will be greatly appreciated!
