Testimonials switched to blog format

I don’t know how this happened but my testimonials went from a very clean format (example: https://modulemaster.com/rebuilds/testimonials/) to a cluttered blog format (example: https://modulemaster.com/rebuilds/testimonials/brakes/) apparently on their own.
The “clean” format uses the shortcode: [testimonials per_page=”5″] whereas the blog formatted ones use a specific category in the shortcode such as: [testimonials category=”1864″ per_page=”5″]
Also, the shortcode: [testimonial-submission-form] no longer works and returns a 404 error.
I’m afraid if I remove and reinstall the plugin, I may loose all the testimonials I had to manually enter (hundreds). I’d back up the database table but could not find it. What is the name of the table used?
