Testimonial Form is not submitting

On this page: https://home-services06.com/evaluez-la-qualite-de-notre-intervention/?nonitro – we’re using testimonial form. When trying to submit it, nothing happens, and console shows JS error with starfish-testimonial.js file. The error does not specify anything, just showing the error.
We’re using NitroPack on the website, but the error is still there when NitroPack is off. In the URL make sure to use ?nonitro parameter in the end, this will bypass NItroPack caching.
WordPress version: 6.4.3
