Test plugin

    Does anybody knows a good test plugin, i am searching for ages but cant find a good one.

    First it need to be a personalia fill in and after that you can answer the questions.

    The best thing would be if you can split in categories to and it shows how far you are with the test in %. Doesnt matter if it is a premium plugin or not. It is for a fitness coach to make dietplans with the answers customers give.

    have looked for surveys and quizes but dont need a result at the end i only need a filled in test what will be send to the admin who will make a personal plan with this info. I can make a contact form ofcourse but a nice test wit next button per categorie will be nice.

    use contact form 7, you can create a pretty fully featured form with checkboxes, radio buttons, dropdowns, text boxes and even media/file uploads. if there’s no need to create a final result based on the information they submit then a nicely built and styled form should do the job. ??

    Yes that was my last option if there was no good plugin for it!
