Test email works, but no others

I have set this up with my host’s SMTP details but have been unable to get it to work. I use it with my WPform.

My host has logged into the accoutn and cannot see why this is.

They have asked me to find out if the contact form has any special requirements and should it use specific php version, are there any specific settings that need to be used?
