TDO Mini Forms Builder Needed

I’ve installed TDO Mini Forms to create a submission form for a coupon site. I’m not familiar with this program, so there might be better options out there. For now, I’m looking for someone with experience using TDO Mini Forms or similar to build me a submission form for a small (affordable) fee. This is a very basic form, which consists of a title, expiration date, an image upload button, price, catagory, and URL. Anyone that can assist would be appreciated.
