Several plugins can be used here to build a page like this.

The options to choose your size and choose your box style can be achieved with the use of variable products. Here’s more about it: https://docs.woocommerce.com/document/variable-product/.

The quantity selection of the items and additional items (gift note and checkboxes as shown in the image here) can be done using the Product Add-ons extension.

Also, it depends on your theme how you want it to look like.
