Table of Contents used to work, now not showing

We use Easy Table of Contents to display a List of Headlines on our Blog pages, it used to look like this:

Since last week the TOC doesn’t show anymore.
I have tested on our stage with the default theme but still no TOC:

Any ideas how we can get it back? it used be auto inserted into posts, here are our setting:
