Support Form Not Showing In Members Area. Help!

I have successfully downloaded and installed this plug-in. It seems like exactly what I need. However, after completing the installation process and attempting to test it out, the Support form isn’t showing up when a member clicks on the “Support” tab. It is showing like this ==> https://goo.gl/M7NA6j
Any ideas? What am I missing?
