Suggestions needed for Waiver for Non Profit

  • Hello! I run a website for a non-profit trapeze school. We would like to have an online waiver that folks can (legally binding) sign and add their emergency contact. In a perfect world there would also be a checkbox to subscribe to our MailChimp newsletter.

    The budget is really low. Anyone have plugins to suggest?
