SUGGESTION NEEDED: Form plugin that shows results on next page and can print

    Hi, I’m looking for some suggestions on which form plugin I should use.

    The only form fields would be First/Last/Email/Phone. I want the results to show on another page after hitting Submit inside of a template and then lets you print or pdf download that template with the form info. What the form is doing is plugging the user’s info into a prescription savings card that is premade with info on it, so it would just be a custom page with blank spots for the user info. After it shows their card, I need to have the function to print and/or download as pdf, I know there are other plugins that do this.

    I’m having the most trouble with the form part. Most forms end with the typical “Thanks for submitting!” message. I’m looking for one that let’s you print the form results into a premade template.

    Maybe there’s a plugin already made for this, maybe there’s one pretty close that I could tweak a little. What are your suggestions?
