Subscription Based Form?

Hello all,

I am doing a bit of testing on this site. I am using Gutenberg. I wanted to be able to do the following (to test).

I want the user to be able to choose three options: Option A, Option B, Option C.

Once they choose, they have to subscribe paying 1 dollar per month

I want to be able to receive that feedback so that I can send them, via email, the option they chose (lets say in a PDF).

what would be the best steps to go on about this?

I guess it would not be too different that ordering pizza online only they subscribe and then after I manually send them what they paid for (document) to their email.
