Submitting subsequent form entries only updates not insert

  • Hello,

    I have a really weird issue where a new app (form) created in DB toolkit inserted as shortcode into a wordpress page (the only thing in the page) only updates the last submitted record and doesn’t insert on all subsequent entries using the form. The “reset” button does not work as well to clear data.

    The reset button works until I actually submit an entry. Then it no longer works after submitting. I have another form on another page working great (WP 3.7). The table both are tied to are setup identically.

    Any ideas?

    I wanted to clarify something I said above….the form apps I am using are tied to different tables and not the same one.
