Submit Review Form Not Working

  • The Rich Reviews Plugin was working just fine as recently as April as we received a review via the plugin. Now it’s a bit broken: https://www.hjacks.com/review-us/, however we have not edited the page and the shortcode is in place as it always has been. The plugin and WP are up to date. Any ideas on why the form is not display and only the stars and an unlabeled submit button? When I choose a star rating and click the button, an empty review is submitted, so that part is connected and working. The form is not displaying however. Thanks in advance for your help. —Del

    Since all of the fields except the stars and submit button are optionally displayed, I’m going to assume that something happened with your database recently that likely reset all of your options to false. I would first go into the Rich Reviews Dashboard and make sure that under form options, all of the fields for the form have the checkbox for display selected.
