I checked your form and I could replicate the issue. After subscribing and being redirected to the CAPTCHA page, I entered the code and got redirected to the /false page with a 404 error.

I saw the request URL for this page was https://translate.googleapis.com/element/log?format=json&hasfast=true&authuser=0

Can you please let us know if you’re using any Google translating plugin on your website? Have you tried to run a plugin/theme conflict to rule out other plugin is affecting this process?
