    Could it be a problem with my prophoto theme

    That is the most likely explanation. You can verify that by temporarily switching to an unmodified, default WordPress theme such as Twenty Sixteen and see if this problem persists.

    Yes, I did that and confirmed it’s a prophoto issue. Awesome Surveys works fine with the default WordPress theme. Thanks!
