Submission form?

  • I have a wordpress hosted with my own host and i would like to add a submission form to my website.

    I need that people could attach images too and that i could choose a maximum allowed size of the image, also maximum allowed number of images.Also i want to have an option that i could publish submitted form or not. I hope you got the idea, what i need.

    Any ideas about this? I should install a plugin? if yes, which plugin i should choose?
