strange behavior adding/editing/selecting custom taxonomies

So I’ve added a custom post type (track) and added a custom taxonomy (track subject) attached to it.
Here’s the strange thing: When I either edit/update/or create a new post from the custom post type and try to assign a custom taxonomy value to it by selecting one of the checkboxes, it selects all.
EX: I create a new track and assign one track subject to the post, it selects all track subjects.
EX 2: I attempt to edit the post by un-ticking one of the track subjects, when I hit edit, it does not remove the track subject I was attempting to delete and keeps all selected.
