Still Looking for the Right Contact Form

I’m building a website for my insurance company and I need a contact form for requesting quotes. I don’t need any kind of live quote implements, just a conditional logic style form that sends me an email so I can just copy paste their information into our rater software.
For example, if someone selected they had 3 cars, I’d like 3 “required input fields” to come up so they can put in the cars information. It would be even better if they could select what coverage amount they wanted under each individual car.
It would also be nice to have a (?) next to some of the fields to further explain what they should input.
A free version would be nice but I’m willing to spend the money if the plugin has everything I need.
