Step-by-step flowchart plugin

I’m looking for a plugin that will allow me to lead the user through a series of questions, kinda like a flowchart, leading the user to a resulting page.

Much like this example flowchart

I was considering just doing a series of pages that were linked, but loading a page after each question could be a pain.

Any ideas or suggestions?
