It seems that your site is under Brute Force Attack.
This is not related to clean-login plugin.

There are a lot of plugins to prevent this attack.

I cannot recommend any plugin as I never used such plugin.

I don’t know if that’s the case for me though. I have someone who logs in successfully but while he’s logged in it still shows failed multiple times.

ULH shows he logged in at 1:29pm, says he is currently logged in but it shows a red icon, then 10 instances of ‘failed’ – not sure why, most recent one was at 3:05 pm.

what does the red icon mean?

If someone is logged-in successfully and still it shows multiple failed for that username, the reason could be that someone else is try to login with the same username but with incorrect password.

The red icon is for:
1) User who logged out
2) User who is inactive for a long time. This is based on settings.
