    Frankly, I don’t understand why. Isn’t it understood that a testimonial may be published? Isn’t that the point? Otherwise, it’s just a contact form. Why add a step to it?

    I think an explicit statement at the bottom like in this demo makes it simpler for everybody.

    Since we’re working in healthcare, HIPPA requirements require us to have a legal release form when using patients names, words, or photos. Do we have control over the explicit statement at the bottom?

    I understand. I will add it to the feature request list. But that sounds outside the scope of a simple checkbox.

    Just add the statement like normal content in the page editor below the [strong] shortcode. You could also add a link to an uploaded document like your release form, or your Terms of Servce or Privacy Policy pages.
