Star ratings not showing on review form

Hi,
Star ratings are displayed normally with our plugin, we supposed your problem is because of some conflicts between your plugins. You can deactive the Photo Reviews plugin to check it. If your problem is solved, it maybe caused by our plugin, please give us your url to check it. In another case, if your problem still exist after deactivating our plugin, it maybe caused by you theme, please contact to your theme developer to fix it.

Will try to disable some. Just updated more than 30 plugins, but no change.

Got star ratings to show by disabling the reviews tab in my theme settings. Reviews was showing up in 2 places, but needs to show up in one place per product page.
