Splitting the input areas across a page?

  • I was hoping to ask if there was a way to “split up” the contact form along a page?

    Meaning if I had:

    Video 1
    Question 1
    Question 2
    _____________________________
    Video 2
    Question 3
    Question 4
    _____________________________

    Video 3
    Question 5
    Question 6
    _____________________________

    Video 4
    Question 7
    Question 8
    _____________________________

    SUBMIT BUTTON

    And then Questions 1-8 would be sent together into the inbox?

    Is this possible?
