Spiritual Assessment using a Likert

  • I am new and trying to create a spiritual assessment using Gravity Form Likert. I am having an issue with the gathering of the information that is being sent to the member. I need it to look something like this below, to show their highest total based on the questions they replied, Always true and Often true to. Please email me at [email protected]

    4 – Often true
    3 – Sometimes true
    2 – Seldom true
    1 – Never true
