Sorry to hear you are having this issue.

I can see your form is on the live payment mode, could you please enable the debug mode following this guide https://wpmudev.com/blog/debugging-wordpress-how-to-use-wp_debug/ in case any extra user reports the same issue, check the wp-content > debug.log file for any Forminator line.

Note, we don’t suggest keeping the debug mode enabled for too long.
