Site title showing up twice on browser header.

Hi,
just trying to correctly configure your great SEO plugin.
Got a problem: as per topic, Site title showing up twice on browser header.
How can I fix it? I have found a two years old post about this, but it’s not very clear. Should I modify my “header.php” or “functions.php”, as stated, is there a better way to do this?
Many thanks in advance!
P.S. I’m also trying which is the best canonicalization setting. It seems it’s not so important which one you choose. Can you give me your opinion, www or not-www?
