[Plugin: Yoast SEO] Site title displays "Comments On"

When doing a Google search for one of my sites, Avera Companies, the title of the site shows up as “Comments On” and the meta description is nonexistent. The titles and descriptions for sub pages/posts show up correctly so I am assuming it is just a problem with the home page. I am using the Yoast SEO plugin.
How can I get the title and description I entered for the home page to display on Google?
