Thanks for your support.
I using wp 4.4 and captcha 4.1.6 . Since i updated the plugin to this version, comments form not load (name and email field load but captcha form and below like textbox not load).
Also footer not load And in wp-login.php , captcha and below (buttons) not load.

So, All things below the captcha not load.
I disabled the captcha plugin and problem fixed.

I use w3tc also.

Perhaps, we can help you to solve your problem. Please activate Captcha plugin for a while and provide a link to your site, so that we could test the plugin work. If necessary, we’ll add some changes to the Captcha code and you’ll be able to use our plugin again.
