Single form for two child pages

Hi,
i want to use the form in bootstrap modal.my parent page has two child in the same page(e.g:https://cmsonlive.com/teamtech/solutions/cloud-and-collaboration/)this page has two sections Polycom and skype for business two are child pages of cloud-and-collaboration .there is forms added as popup for CTA buttons.now am added a forms in two pages .

but i want to add only in template and the subject of the each form should be that title of child pages. i tried like that am not getting the exact title, am getting only first child page title . how to get separately in form field.
