SimpleModal Contact Form Error

  • I installed the SimpleModal Contact Form Plugin. All worked perfectly fine and well.

    Once I turned on my permalinks the link doesn’t work anymore.
    If I turn off the JQuery option I get a message that the page cannot be found.

    Any ideas…?

    I went to the plugins homepage and another user there had a similar error.

    The author said that you should make sure to include this code because I modified my theme:

    <?php wp_footer(); ?>
