simple-proxy is not working!

Hi.
i downloaded and installed your “simple-proxy” plugin.
config it all with your directions but after i save my settings my website is going look like this:(plugin is enable at the moment)
tested in 2 different hostings and one VPS .
it’s all the same.
please help.
