Simple Pro Quiz Form

Hi, anyone has any idea if there’s a plugin that helps with this?

(First window)
Form that asks for email address with “submit email add” button

Once email is submitted, the quiz starts:

Question sample 1: Are you a female?
Yes (Button)
No (button)
(user is to click on either one)
Once it’s clicked, next page with next question appears, and so on.

Steps are also shown like this (2 our of 15 questions answered, with completion bar)

Once complete, a page appears with results like 10 out of 15 for example.

Shortcode available to embed in custom pages.

I guess there’s no need for a quiz plugin as long as there is anything that can serve this purpose will be fine.

I’ve been looking high and low and trying all the quiz plugins out there but all are not as simple as this.
