Simple list plugin

Hello everyone,

I’m looking for a plugin to do the following:

Make a simple list with name, predefined group (dropdown list) and date (preferably with jquery datepicker, but not required). Anyone know of a plugin that can do this? Registered users need to be able to add and remove entries at will
