Simple email notification plugin.

I have come across quite a few email notification plugins but I want something more simple…

1. a form for just an input box and a subscribe button.
2. submitting the form will take you back to the original site and not another page with a bunch of options.
3. a verfication email is send out.

I searched the plugin lists but could not find something as simple as what I requested… if such a plugin exists do point me in that direction.
