@brian12988, @simonduchaine thanks for reaching out and I apologize for the late reply. Sometimes with the pop-up form, it takes a hard reset of the cache for things to show up (which explains why it worked on another device). It sounds like you all are on the right track, but please let us know if you have any other issues.
