Sign Up Form stopped showing uip

Website: Sonshinebarn.com has the Newsletter sign up on the Home page toward the bottom. After Wedding Portfolio and before What’s New, sections. This was always loading, so unsure why it stopped now.

It’s connecting with Mailchimp since when I clicked on renew Mailchimp lists, it renewed and pulled up the correct list.
