    The initial form loads, but after you click on one of the “Sign Up” links, the form doesn’t load so you can’t complete the sign up.

    That’s not much info to go by, and I assure you the plugin works correctly.

    You either have some configured wrong, or there is a conflict with another plugin or theme.

    Where is the initial form? I inspected the page for the link you sent, and don’t see any of the output divs or other html elements my plugin generates, which makes me think you do not have the shortcode on the page that you linked to.

    Without being to see how you have things set up on the admin side, I really can’t help you. Not enough info for me to know what’s going on.

    Apologies, I put it in test mode during trouble shooting. You should see the form now.

    We’ve been using this plugin for 4 years and in the last week, the link through to input info into the sign up has stopped working.

    I’m sure there is a conflict with something but I can’t figure it out.

    Can’t really tell what’s happening. Could be some caching going on. I still didn’t see the list of sheets until I held down shift and reloaded the page a couple of times.

    If you have a caching plugin, make sure you disable it on any page like this which has dynamic content.

    If that doesn’t work, maybe go to your permalinks page (admin settings for WordPress) and just hit save again to clear any weird permalink issues.

    Otherwise, it’s hard for me to figure out what’s going on. If you’re using some kind of block editor, visual editor, in your theme or plugin, maybe try putting the shortcode in a different kind of block (like a pure HTML block).

    At this point, I’m just guessing, without being able to see your admin setup and what else is going on.

    If you installed some new plugins, maybe try deactivating those one at a time and rechecking the page, to see if you can find a possible conflict.
