Sign In Form For WP- Pro Quiz

Hi Guys,

I recently installed WP-Pro Quiz on my wordpress site. I created the quiz on the development site. However when I launch my site I get a respond saying “You must sign in or sign up to start the quiz.” but there is no sign up or sign in form on the page. How do I get the signup form to appear on my page? I would really appreciate your feedback.
