Shortcode stopped appearing on page

Shortcode was working fine yesterday and uploads worked great. Upload form doesn’t appear on the page today. The only change I made was to check the setting to use the alternate Iptanus server because I get the error message:

Iptanus Services Server is unreachable. This may cause problems on some plugin functions.

Once I saw that the shortcode wasn’t working though, I unchecked the alternate server setting, but the shortcode still isn’t working.
