Shortcode input fields don’t display after latest update (1.4.5)

    Hi,

    After the latest update of this plugin (1.4.5) the shortcode input boxes are not appearing any more. Here’s the problem with YouTube shortcode selection: https://prnt.sc/fb7bfr
    It is the same with other shortcode selections too.

    Please fix this bug.
