    Hi, yes I have a client that sells mugs etc. with the last name on the mugs etc. I need to add a form field on the product page so the customer can add their last name.

    Hi,

    If you go to the bottom link you will see 2 form field boxes on that product page that say “Last Name :” and the other says “Country of Origin (if known) :”, that is what I need to accomplished for my new wordpress store, that’s why I was asking if your plugin supplies a shortcode for form fields. Thanks.

    I have the same question. I currently have a paypal generated button to make a payment for a class and it allows me a form field for the student name. This allows someone to make a payment for several students at once. e.g. <input type="hidden" name="on1" value="class participant name">Enter the Class Participant's name you are paying for:</td></tr><tr><td><input type="text" name="os1" maxlength="200">
