Shortcode Builder Not Making Shortcodes

I’m using the plugin with Gravity Forms. I submitted a couple of form entries, and I can see them displaying on the Contact Form DB screen, so the data is coming in. When I go to create a shortcode, the shortcode is not generated/displayed. I do get some dropdown choices under “select a shortcode” but when I select one, nothing happens. In the tutorial video it shows an option to choose your form at the top of the shortcode page, but this option is not visible for me at all.
