shortcode and script compatibility with GuiForm plug-in

I have a plugin named GuiForm and when I activate Restricted Site Access and use the shortcode for the form [GuiForm id=”1″] it now displays the site home page where the form should be. When I use the embed script in lieu of the short code <script type=”text/javascript” src=”https://www.immunizationsandhealthit.org/form/js/1″></script> then nothing displays on the page.
If I deactivate Restricted Site Access both of these code methods work perfectly.
Any ideas?
