Thanks for sharing the screenshot of your flat rate shipping settings and your System Status.

I performed a cross-testing on one of my test sites and your formula for five items is working correctly, as you can see on the images below:

I’m running a basic configuration though with only the official Storefront theme and WooCommerce active.

Can you please run a conflict test and see if that helps solve the issue?

• Temporarily switch your theme to Storefront
• Disable all plugins except for WooCommerce
• Repeat the action that is causing the problem

If you’re not seeing the same behavior after completing the conflict test, then you know the problem was with the plugins and/or theme you deactivated.

Reactivate the theme first and check again. Then, go on and reactivate your other plugins one by one, testing after each, until you find the one causing conflict.
